Rapid and sustained antidepressant effects of vaporized N,N-dimethyltryptamine: a phase 2a clinical trial in treatment-resistant depression

This open-label fixed-order dose-escalation trial (n=14) evaluated inhaled DMT (15mg & 60mg) for treatment-resistant depression (TRD) for the first time. Results showed rapid and sustained antidepressant effects with a 21-point reduction on the Montgomery-Asberg Depression Rating Scale by day 7 (p<0.001), an 86% response rate, and a 57% remission rate lasting up to 3 months, with significant decreases in suicidal ideation (SI).

Comparing Antidepressant Effects of Psilocybin-Assisted Psychotherapy in Individuals That Were Unmedicated at Initial Screening Versus Individuals Discontinuing Medications for Study Participation

In an open‑label trial of 27 treatment‑resistant depression patients given a single 25 mg psilocybin dose with psychotherapy, those who had discontinued antidepressants before treatment showed comparable reductions in clinician‑rated and self‑reported depression, anxiety and suicidality to patients unmedicated at screening. Both groups experienced clinically significant improvements and similar intensity of psychedelic experience, suggesting medication tapering did not alter short‑term antidepressant effects in this sample.

Right dorsolateral prefrontal cortex volumetric reduction is associated with antidepressant effect of low-dose ketamine infusion: A randomized, double-blind, midazolam-controlled PET-MRI clinical trial

This double-blind, randomised study (n=48) involving patients with treatment-resistant depression (TRD) and suicidal ideation (SI) evaluated the effects of a single infusion of 35mg/70kg ketamine or 3.15mg/70kg midazolam. Using positron emission tomography-magnetic resonance imaging, the study observed a small but significant volumetric reduction in the right dorsolateral prefrontal cortex (DLPFC) in the ketamine group compared to the midazolam group, and a greater reduction in depressive symptoms was associated with smaller decreases in right DLPFC volumes.

A Randomized Double-Blind Midazolam-Controlled Trial of Low-Dose Ketamine Infusion in Patients With Treatment-Resistant Depression and Prominent Suicidal Ideation

In this randomized double‑blind trial of 84 outpatients with treatment‑resistant depression and prominent suicidal ideation, a single low‑dose ketamine infusion (35mg/70kg) produced greater antidepressant effects than midazolam lasting up to 14 days and antisuicidal effects lasting about 5 days. Benefits were most evident in patients whose current episode was under 24 months or who had failed ≤4 antidepressants, and the infusion was safe and well tolerated.

A Pilot Study of Ketamine Infusion after Suicide Attempt: New Frontiers in Treating Acute Suicidality in a Real-World Medical Setting

In an open‑label pilot of 16 medically hospitalised recent suicide attempters, a single intravenous ketamine infusion (0.5 mg/kg) produced rapid, large reductions in suicidality and depression at 24 hours and 5 days (Cohen’s d 1.7–8.8) that were sustained up to six months. The study indicates ketamine is a feasible, rapidly acting intervention for acute suicidality in real‑world Consultation‑Liaison settings.

The effect of single administration of intravenous ketamine augmentation on suicidal ideation in treatment-resistant unipolar depression: Results from a randomized double-blind study

This randomised double-blind placebo-controlled study (n=56) found that a single dose of ketamine significantly reduced the level of suicidal ideation (SI). Still, in both groups, there was a high level of suicidal ideation, and the study showed diminishing results rapidly over the 30 days of the study (the effects being the largest at 3 days post-infusion).
