An open-label, dose-escalation trial of psilocybin-assisted therapy for bipolar 2 depression

In an open-label, single-arm dose-escalation pilot (n=14) of psilocybin-assisted therapy for bipolar II depression, psilocybin (10 mg, with 25 mg if needed) was generally well tolerated and produced significant, sustained reductions in depressive symptoms and improved quality of life up to 90 days, with adverse events (including transient anxiety, nausea, headache and three notable psychiatric events) broadly comparable to other psilocybin studies. The authors conclude that randomised, placebo-controlled trials are needed to confirm efficacy and refine dosing protocols in this vulnerable population.

Single-Dose Synthetic Psilocybin With Psychotherapy for Treatment-Resistant Bipolar Type II Major Depressive Episodes

In an open‑label, non‑randomised 12‑week trial of 15 patients with treatment‑resistant bipolar II depression, a single 25 mg dose of synthetic psilocybin plus psychotherapy produced large, rapid and sustained reductions in depressive symptoms (mean MADRS change −24 points at 3 weeks; 12/15 responders, 11/15 remissions) without significant increases in mania or suicidality. These preliminary results suggest psilocybin may be an effective and safe acute treatment for bipolar II depression and merit confirmation in larger randomised controlled trials.

Effect of Repeated Intravenous Esketamine on Adolescents With Major Depressive Disorder and Suicidal Ideation: A Randomized Active-Placebo-Controlled Trial

This placebo-controlled trial (n=54) investigates the efficacy and safety of esketamine (iv, 17.5mg, 3x) in adolescents suffering from depression (MDD) and suicidal ideation (SI). It shows significant reductions in Columbia Suicide Severity Rating Scale (C-SSRS) Ideation and Intensity scores and Montgomery-Åsberg Depression Rating Scale (MADRS) scores in the esketamine group compared to the midazolam (placebo) group at day six, with maintained antisuicidal and antidepressant responses at four weeks post-treatment.

Nitrous oxide as an adjunctive therapy in major depressive disorder: a randomized controlled double-blind pilot trial

This double-blind placebo-controlled between-subjects study (n=23) tested the antidepressant efficacy of inhaled nitrous oxide (50% N2O|50% O2 versus 100% O2) in patients diagnosed with major depression (MDD). Across multiple treatment sessions administered across a period of 4 weeks, there were significant reductions in depressive symptoms in the acute response to treatment and accumulatively across sessions.

A single infusion of ketamine improves depression scores in patients with anxious bipolar depression

In a post‑hoc analysis of 36 treatment‑resistant bipolar depressed patients (on lithium or valproate), a single 0.5 mg/kg ketamine infusion produced significant antidepressant effects on MADRS and HDRS through 14 days in both anxious and non‑anxious subgroups, with no evidence that baseline anxiety reduced ketamine’s efficacy.
