Expert Opinion on Psychedelic-Assisted Psychotherapy for People with Psychopathological Psychotic Experiences and Psychotic Disorders

This expert consultation study (n=12) investigated the exclusion of individuals with personal or familial histories of psychopathological experiences from most psychedelic clinical trials and treatment programs. Experts in psychiatry, clinical psychology, medicine, and psychedelics were interviewed, and the findings revealed that exclusion criteria may be justified in studies with minimal psychological support. They agreed that psychedelic-assisted psychotherapy, as well as therapy with MDMA and ketamine, might be beneficial for some individuals within this group.

Beating Pain with Psychedelics: Matter over Mind?

This review (2021) makes the case for using psychedelics to treat pain. Key areas discussed include studies that have directly used psychedelics to treat pain, potential neuro-restorative effects of psychedelics in pain-related states of consciousness, anti-neuroinflammatory and pro-immunomodulatory actions of psychedelics and the safety, legal, and ethical consideration inherent in psychedelics’ pharmacotherapy. Psychedelics could also help to elucidate the mechanism of pain syndromes.

Low-dose ketamine does not improve the speed of recovery from depression in electroconvulsive therapy: a randomized controlled trial

This randomised, double-blind, placebo-controlled study investigated the efficacy of using low-dose ketamine (35mg/70kg) as an anaesthetic adjunct to propofol during electroconvulsive therapy (ECT) for treating patients with depression (MDD). Adding ketamine did not alter the antidepressant efficacy or the hemodynamics of electroconvulsive therapy, although it may help reduce the dose requirements of propofol anaesthesia.

Antisuicidal and antidepressant effects of ketamine and esketamine in patients with baseline suicidality: A systematic review

This review (2021) evaluates the effect of esketamine and ketamine in patients with suicidal ideation at baseline. The authors find that esketamine trials did not demonstrate antisuicidal effects, while IV ketamine appeared to rapidly decrease the short term severity of suicidal ideation and depressive symptoms in individuals with baseline suicidal ideation.

Ketamine treatment for individuals with treatment-resistant depression: longitudinal qualitative interview study of patient experiences

In a longitudinal qualitative study of 12 patients with treatment-resistant depression, ketamine typically produced short-term improvements in mood and reductions in suicidal ideation, but benefits were transient for most and some experienced increased hopelessness and suicidality as effects wore off. Participants also reported side-effects and cost concerns, prompting recommendations for closer monitoring, adjunctive psychological therapy and explicit informed consent about relapse and potential worsening of suicidality.

A rapid positive influence of S-ketamine on the anxiety of patients in palliative care: a retrospective pilot study

A retrospective pilot study of eight palliative care patients given an intravenous S-ketamine infusion (0.25 mg/kg) found a rapid, significant reduction in anxiety compared with matched controls, with no significant change in depression or lasting analgesic effects. Findings are limited by the small, non-randomised retrospective design and need confirmation in prospective randomised trials.

RTT Ketamine-Assisted Psychedelic Therapy: Indigenous Cohort Quality-Improvement Pilot (Roots to Thrive × Snuneymuxw First Nation, 2023)

This unregistered trial (n=10) was a quality-improvement pilot of ketamine-assisted group therapy for mental health in an Indigenous cohort, which reported significant perceived benefits and highlighted the importance of cultural safety.
