Phenomenology and therapeutic potential of patient experiences during oral esketamine treatment for treatment-resistant depression: an interpretative phenomenological study

This qualitative interview study (n=17) of those receiving esketamine (35-210mg/70kg, 12x over 6w) for depression explores patients' experiences. Findings reveal highly variable effects of ketamine with common psychological distress, and central themes include perceptual effects, detachment, stillness and openness, mystical-type effects, fear and anxiety, feeling hungover and tired, and a neutralizing mood effect post-session. While patients reported several esketamine effects with psychotherapeutic potential such as increased openness, detachment, and mystical-type experiences, the study identifies a need for additional support due to the frequency and severity of the perceived distress during the treatment.

Hallucinogenic drugs and their potential for treating fear-related disorders: Through the lens of fear extinction

This comprehensive review argues that hallucinogenic drugs can enhance fear extinction—by promoting neuroplasticity, tempering amygdala hyperreactivity and facilitating emotional engagement—and thus offer advantages over current first‑line treatments for fear‑related disorders. It highlights MDMA and ketamine as the most promising therapeutics to augment exposure therapy and examines the molecular mechanisms relevant to hallucinogen‑assisted psychotherapy.

Repeated intranasal ketamine for treatment-resistant depression - the way to go? Results from a pilot randomised controlled trial

In a halted double‑blind pilot RCT of repeated intranasal 100 mg ketamine for treatment‑resistant depression, acute cardiovascular, psychotomimetic and neurological side effects and marked interindividual variability in ketamine/norketamine absorption produced poor tolerability and impractical administration, with no clear efficacy advantage over midazolam. Using the formulation and delivery device tested, intranasal ketamine was not a viable treatment approach, although no hepatic, cognitive or urinary adverse changes were observed.

This article (2015) examines the advantages and applications of intranasal drug delivery, with a particular focus on the potential of intranasal ketamine for the acute and maintenance therapy of refractory depression. The article contrasts intranasal delivery to oral and sublingual delivery methods, which are less effective with regards to their bioavailability, crossing of the blood-brain-barrier, and rapid onset of drug effects.

Ketamine Psychoactive Effects in Cocaine Dependence: Double-Blind 3-Way Crossover Pilot (Dakwar / Nunes, NYSPI Columbia 2014)

This unregistered trial (n=8) was a double-blind, 3-way crossover pilot of ketamine infusions for cocaine dependence in cocaine-dependent adults, finding that acute mystical-type effects mediated improvements in the motivation to quit cocaine.
