An open-label, dose-escalation trial of psilocybin-assisted therapy for bipolar 2 depression

In an open-label, single-arm dose-escalation pilot (n=14) of psilocybin-assisted therapy for bipolar II depression, psilocybin (10 mg, with 25 mg if needed) was generally well tolerated and produced significant, sustained reductions in depressive symptoms and improved quality of life up to 90 days, with adverse events (including transient anxiety, nausea, headache and three notable psychiatric events) broadly comparable to other psilocybin studies. The authors conclude that randomised, placebo-controlled trials are needed to confirm efficacy and refine dosing protocols in this vulnerable population.

‘Equal-unblinding’ meta-analysis of psychedelic therapy vs. antidepressants for the treatment of depression

In a pre-registered meta-analysis of 8 PAT trials (548 patients) and 16 open‑label traditional antidepressant studies (9,751 patients), psychedelic-assisted therapy was not more effective than open‑label antidepressants on the 17‑item Hamilton Depression Rating Scale (difference 0.3 favouring tAD; 95% CI −1.39 to 1.98; p=0.73). This equal‑unblinding comparison emphasises that blinding integrity materially influences apparent treatment effects and that both PAT and tAD produced robust, clinically meaningful improvements.

Rapid and sustained reduction of treatment-resistant PTSD symptoms after intravenous ketamine in a real-world, psychedelic paradigm

In a real-world sample of 117 treatment‑resistant PTSD outpatients, intravenous ketamine delivered within a supportive “psychedelic” paradigm (preparation, music, integration and psychotherapy) was associated with large, rapid and sustained reductions in PCL‑5 scores (mean 52.5 → 28.8, d = 1.64), with 75% showing clinically meaningful improvement and 62% meeting criteria for remission. Treatment was well tolerated with no serious adverse events, suggesting this delivery model may enhance ketamine’s effectiveness for refractory PTSD.

Salience, Sensemaking, and Setting in Psilocybin Microdosing: Methodological Lessons and Preliminary Findings of a Mixed Method Qualitative Study

Using momentary ecological assessment and retrospective interviews, the study found psilocybin microdosing was associated with loosening of mental structures, increased salience of external stimuli, greater cognitive flexibility but reduced cognitive stability and occasional ego-dystonic content, with highly structured environments tending to produce more positive appraisals. Crucially, momentary and retrospective reports were often diametrically opposed, highlighting methodological biases and the need for systematic mixed-methods studies to accurately characterise the lived experience of microdosing.

Psilocybin-assisted neurofeedback for the improvement of executive functions: a semi-naturalistic-lab feasibility study

This semi-naturalistic feasibility study found that psilocybin-assisted frontal-midline theta neurofeedback is practicable without psychological support and produced a trend-level increase in frontal-midline theta alongside significant self-reported improvements in everyday executive functions (working memory, shifting, monitoring, inhibition). However, no objective task-based gains were observed, indicating the need for larger randomised trials with regular sessions and active controls to confirm efficacy and specificity.
