In an open‑label Phase IIa trial (n-12), a single 10 mg intranasal dose of 5‑MeO‑DMT (BPL‑003) given alongside a 10‑week relapse‑prevention CBT programme was acceptable in safety and tolerability in people with moderate–severe alcohol use disorder. Over 12 weeks, participants showed large improvements in drinking (mean abstinent days rose from 33.2% to 80.8% and heavy drinking days fell from 56.2% to 13.2%), providing preliminary evidence of efficacy and supporting larger controlled trials.

Percentage of Heavy Drinking Days Following Psilocybin-Assisted Psychotherapy vs Placebo in the Treatment of Adult Patients With Alcohol Use Disorder

In a double‑blind randomised clinical trial of adults with alcohol dependence receiving psychotherapy, two high‑dose psilocybin sessions reduced the percentage of heavy drinking days over 32 weeks to 9.7% versus 23.6% with active placebo (mean difference 13.9 percentage points; 95% CI 3.0–24.7; P = .01). Psilocybin was well tolerated with no serious adverse events, supporting further study of psilocybin‑assisted therapy for alcohol use disorder.

First study of safety and tolerability of 3,4-methylenedioxymethamphetamine-assisted psychotherapy in patients with alcohol use disorder

In the first open‑label study of MDMA‑assisted psychotherapy for alcohol use disorder (BIMA), 14 post‑detox participants tolerated two 187.5 mg MDMA sessions with no unexpected adverse events and showed improved psychosocial functioning. At nine months average alcohol use fell from 130.6 to 18.7 units per week, supporting safety and signalling therapeutic potential that now warrants controlled trials.

Psilocybin-assisted treatment for alcohol dependence: a proof-of-concept study

In a single-group proof-of-concept study of 10 participants with DSM‑IV alcohol dependence, psilocybin administered alongside motivational enhancement therapy produced a significant increase in abstinence after dosing that was largely maintained to 36 weeks, with the intensity of the first session strongly predicting subsequent reductions in drinking, craving and increased abstinence self‑efficacy. There were no significant treatment‑related adverse events, supporting the need for larger controlled trials to evaluate efficacy and mechanisms.
