5-Methoxy-N,N-dimethyltryptamine (5-MeO-DMT) for alcohol use disorder: An open-label, phase 2, proof-of-concept, clinical trial

In an open‑label Phase IIa trial (n-12), a single 10 mg intranasal dose of 5‑MeO‑DMT (BPL‑003) given alongside a 10‑week relapse‑prevention CBT programme was acceptable in safety and tolerability in people with moderate–severe alcohol use disorder. Over 12 weeks, participants showed large improvements in drinking (mean abstinent days rose from 33.2% to 80.8% and heavy drinking days fell from 56.2% to 13.2%), providing preliminary evidence of efficacy and supporting larger controlled trials.

Complex slow waves in the human brain under 5-MeO-DMT

This naturalistic EEG study (n=29) examines the effects of inhaled synthetic 5-MeO-DMT (12mg) on brain activity in healthy individuals. It finds that 5-MeO-DMT radically reorganises low-frequency neural activity flows, making them incoherent, heterogeneous, and nonrecurring. It also causes broadband activity to exhibit slower, more stable, low-dimensional behaviour with increased energy barriers to rapid global shifts.

Psilocybin-assisted therapy for relapse prevention in alcohol use disorder: a phase 2 randomized clinical trial

This double-blind randomised clinical trial (n=37) found that a single dose of psilocybin (25mg) with brief psychotherapy did not significantly reduce alcohol relapse rates or consumption compared to placebo in patients with alcohol use disorder (AUD) at 4-week or 6-month follow-up, though psilocybin participants reported additional reductions in craving and temptation to drink, suggesting larger trials are needed to evaluate this approach for severely affected patients.

Psychedelic treatment for co-occurring alcohol misuse and post-traumatic stress symptoms among United States Special Operations Forces Veterans

In a retrospective survey of 27 US Special Operations Forces veterans who completed a psychedelic treatment programme in Mexico, self‑reported alcohol misuse and post‑traumatic stress symptoms showed marked reductions with very large effect sizes (d≈–2.4 to –2.8) and 85% reduced drinking to non‑risky levels, while psychological flexibility increased. The uncontrolled, retrospective design limits causal inference, and the authors call for rigorous longitudinal trials to determine whether psychedelic‑assisted therapy is effective in this population.

The epidemiology of 5-methoxy- N, N-dimethyltryptamine (5-MeO-DMT) use: Benefits, consequences, patterns of use, subjective effects, and reasons for consumption

In an online survey of 515 respondents, 5‑MeO‑DMT was used infrequently and primarily for spiritual exploration, producing strong self‑reported mystical-type experiences with relatively few challenging effects and low indicators of addiction or adverse legal/medical outcomes. Many users with psychiatric diagnoses reported self‑perceived improvements (e.g. PTSD, depression, anxiety), suggesting potential psychotherapeutic effects that warrant rigorous human safety and pharmacokinetic studies.
