PTSD

Post-traumatic stress disorder (PTSD) is where psychedelic medicine has its most famous result and its hardest reality check. MDMA-assisted therapy ran the only completed Phase 3 programme for any psychedelic in PTSD, with striking numbers, yet in August 2024 the US regulator declined to approve it and asked for another trial. This page sets the strongest evidence in the field beside the unresolved doubts about blinding, durability and trial conduct that the regulator flagged.

Efficacy and Safety of the Neuroplastogen TSND-201 for the Treatment of PTSD A Randomized Clinical Trial

In a multicentre, double‑blind, placebo‑controlled phase 2 trial of 65 adults with chronic PTSD, once‑weekly oral TSND‑201 produced significantly greater reductions in clinician‑rated PTSD severity (CAPS‑5; LS mean difference 9.64, P = .01) and improvements in self‑reported symptoms, functioning and depression versus placebo. TSND‑201 was generally well tolerated — common adverse events included headache, decreased appetite, nausea, dizziness and transient blood‑pressure increases — supporting its potential as a rapid‑acting, durable treatment for PTSD.

Safety and tolerability of multiple sublingual microdoses of 5-MeO-DMT in adults with moderate symptoms of depression and/or anxiety: a randomized, double-blind, placebo-controlled study

This Phase I clinical trial (n=36) of sublingual 5-MeO-DMT (6-12 mg weekly doses over four weeks) in adults with moderate to high anxiety/depression demonstrated good safety and tolerability with no significant adverse events, rapid absorption with peak plasma concentrations at 20 minutes, dose-dependent neurophysiological modulation without full psychedelic effects, and maintenance of normal cognitive and behavioral function.

5-MeO-DMT for post-traumatic stress disorder: a real-world longitudinal case study

This first longitudinal case study reports that a single inhaled dose of toad‑derived 5‑MeO‑DMT produced rapid, clinically significant and sustained reductions in PTSD symptoms, hopelessness and suicide risk in a 23‑year‑old woman, with the participant endorsing a complete mystical experience and no drug‑related serious adverse events. Improvements persisted for 12 months but were accompanied by acute nausea, overwhelming subjective effects and later night terrors.

Open-label study of consecutive ibogaine and 5-MeO-DMT assisted-therapy for trauma-exposed male Special Operations Forces Veterans

This open-label study (n=86) examined the effectiveness of psychedelic-assisted therapy (PAT) among trauma-exposed Special Operations Forces Veterans (SOFV) in Mexico. Results indicated significant improvements in self-reported PTSD symptoms, depression, anxiety, insomnia severity, post-concussive symptoms, satisfaction with life, psychological flexibility, and cognitive functioning from baseline to one-month follow-up. The combination of ibogaine and 5-MeO-DMT assisted therapy showed potential for providing rapid and lasting improvements in mental health functioning, with effects observed up to six months after treatment.

Predictors of psychedelic treatment outcomes among special operations forces veterans

This prospective study (n=86) examined the effects of ibogaine and 5-MeO-DMT treatment on U.S. Special Operations Forces Veterans with trauma exposure. Younger age and higher baseline levels of depression and anxiety were correlated with significant improvements in mental and psychosocial outcomes from baseline to 1-month follow-up. Greater intensity of changes in consciousness was linked to improved long-term mental health and psychosocial outcomes up to 6 months post-treatment.

Prospective associations of psychedelic treatment for co-occurring alcohol misuse and posttraumatic stress symptoms among United States Special Operations Forces Veterans

This prospective study (n=86) evaluated the effects of ibogaine & 5-MeO-DMT treatment on risky alcohol use & PTSD symptoms among US Special Operations Forces Veterans. It found a significant reduction in alcohol use from pre-treatment to 1-month, 3-months, and 6-months post-treatment, and large differences between responders and non-responders in PTSD symptom and cognitive functioning change, suggesting that psychedelic-assisted therapy may hold promise for individuals with complex trauma and alcohol misuse.
