Psilocybin for Tobacco/Nicotine Use Disorder (TUD)

31 papers and 2 clinical trials exploring psilocybin as a treatment for tobacco/nicotine use disorder (tud).

CompoundClassic Psychedelic

Psilocybin

Psilocybin is a naturally occurring tryptamine psychedelic that acts as a prodrug to psilocin, a potent 5-HT2A receptor agonist. It is the furthest advanced psychedelic in clinical development, with two positive Phase III trials in treatment-resistant depression and expanding regulated access in Australia, Germany, and US states.

Tobacco/Nicotine Use Disorder (TUD)

Tobacco Use Disorder (TUD) presents significant challenges for public health, and emerging research indicates that psychedelic compounds may offer novel therapeutic avenues for treatment. Initial studies highlight the potential of psychedelics like ayahuasca and ibogaine in reducing tobacco dependence.

Psilocybin or Nicotine Patch for Smoking Cessation A Pilot Randomized Clinical Trial

This pilot randomised clinical trial (n=82) found that a single dose of psilocybin (30mg/70kg) combined with cognitive behavioural therapy produced significantly higher long-term smoking abstinence rates at 6 months compared to nicotine patches plus CBT (41% vs 10%), with no serious adverse events attributed to either treatment.

Health-related behavioral changes following the use of psychedelics in naturalistic settings

This cross-sectional study (n=2,510) of US adults with psychedelic experience found that participants retrospectively reported widespread improvements in health behaviours including reduced alcohol (66%) and tobacco (49%) use, better dietary habits (49%), and decreased impulsivity (48-72%), with microdosers and frequent users showing greater positive changes.

Psilocybin for Opioid Use Disorder in Two Adults Stabilized on Buprenorphine: A Technical Report on Study Modifications and Preliminary Findings

This safety-feasibility trial (n=2) explored the interaction between psilocybin and buprenorphine in adults with opioid use disorder (OUD). The study found that coadministration of psilocybin and buprenorphine was safely tolerated, with no serious adverse events or significant changes in opioid craving or withdrawal measures. Feasibility challenges led to modifications in the study population and eligibility criteria, emphasizing the need for improved accessibility and overall generalizability.

Therapeutic effect of psilocybin in addiction: A systematic review

This review (2023, s=4) finds that only 151 patients (and one clinical trial) have been through trials with psilocybin (6-40mg) for addiction (alcohol & smoking). Still, the findings were positive and larger trials are underway.

The association between naturalistic use of psychedelics and co-occurring substance use disorders

Using 2017 NSDUH data (n=56,276), the authors found past naturalistic use of peyote/mescaline was associated with lower odds of past-year substance dependence or abuse (adjusted OR 0.68, p<0.001), whereas past LSD and psilocybin use were associated with increased odds; none of the classic psychedelics were linked to nicotine dependence. The paper notes it is unclear whether these differences reflect pharmacological properties or the cultural/ceremonial contexts of peyote/mescaline use.

Psychedelic Identity Shift: A Critical Approach to Set And Setting

This analysis of the therapeutic frameworks used in psychedelic-assisted treatment (for smoking cessation specifically) finds that suggestions from the framework map onto outcomes (and the language used by participants) from the study. This has broader implications for psychedelic-assisted therapy, as suggestions (in the therapeutic framework) can be used for various purposes (positive and negative).
